Atrium Healthcare are recruiting on behalf of a client for Community and Inclusion Lead. The role is responsible for working with the Community and Social Inclusion Manager and the rest of Core Group, to ensure that Registered Managers and wider management teams in the home/school are supported to run their homes/schools in a way that supports Community & Social Inclusion needs.
 
Main responsibilities and duties of the job:
 
·Work closely with the Community and Social Inclusion manager to develop and embed this vision.
·Develop and maintain links to external professional groups, forums and boards.
·Source, research and apply for any engagement grants or funding opportunities.
·Travel to and visit placements to risk assess and secure relationships and the suitability of placements.
·Once placed, visit the adults within the placements to coach and mentor the Support Workers to be able to retain and sustain the placements.
·Liaising with Community & Social Inclusion manager and or the school to ensure the careers pathways are implemented and retained.
·Ensure quality is measured and feedback is obtained to support the continued development.
·Maintain knowledge of relevant legislation and key documents to support Social Inclusion.
·Design and deliver workshops to support the development of knowledge around Community and Social Inclusion staff teams.
·Develop and deliver Easy Reads as required.
·Ensure training and policy reading is up to date.
 
Desirable:
 
Experience working within a role that includes the following:
·Event planner and organization.
·Activities coordinator.
·Experience within social care/education sectors.
·Knowledge and experience of person centred care.
·Experience and knowledge of safeguarding Adults and Children.
 
Essential:
 
·Previous experience of working in a residential adult setting.
·Previous experience of working with young people/adults with SEND.
·Level 3 in Health and Social Care.
·Must hold a full clean UK driving license - travel between sites is essential.
 
Hours of work:
 
32 hours per week (Monday - Friday some evenings with notice).
